There is lots of talk concerning the need for model releases. Often the verbal approval of a parent is sufficient but if you wish to put this photo on a business card, in your literature or in your book of photos for the child to choose from you really need to get a legal release. There are many approaches. This is the example shared by Shannon Fennell from Canada. You are welcome to use hers or modify it to your liking. When in doubt seek legal advice as we are not legal experts.

For valuable consideration, receipt of which is hereby acknowledged, I hereby grant <company name> the irrevocable right and permission, throughout the world, in connection with the photograph(s) that were taken of me or which I provided to <company name>, (copies of which photographs are annexed hereto and made a part), the following: the right to use and reuse, in any manner at all, said photographs, in whole or part, modified or altered, either by themselves or in conjunction with other photographs, in any medium or form of distribution, and for any purpose whatsoever including, without limitation, all promotional and advertising uses, and other trade purposes, as well as using my name in connection therewith, if <company name> so desires.
I hereby forever release and discharge <company name> from any and all claims, actions and demands arising out of or in connection with the use of said photographs, including, without limitation, any and all claims for invasion of privacy and libel.
